Fatality Assessment and Control Evaluation (FACE) Report: Tower Hand Dies After211 230-Foot Fall from Communications Tower, North Carolina, November 13, 1998

On November 13, 1998, a 40-year-old male tower hand (the victim) died after211u001efalling 230 feet from a 250-foot communications tower. Office of the North 211u001eCarolina Occupational Safety and Health Administration (NCOSHA) notified the 211u001eDivision of Safety Research (DSR) of this fatality. DSR offered technical 211u001eassistance and on December 15, 1998, a safety specialist from DSR conducted an 211u001einvestigation of the incident. The incident was reviewed with the NCOSHA 211u001ecompliance officer assigned to the case, the tower-owner representative, and the 211u001ecounty sheriff and coroner offices. The site was visited and photographs were 211u001eobtained during the investigation.
