Interactive, Guideline-Based Decision Support on the Web. Abstract, ExecutiveSummary and Final Report

Clinical practice guidelines are touted as a mechanism for translating researchinto practice to improve healthcare, but the benefits of guideline-based care have not been realized in part because of significant barriers to widespread guideline utilization. We have developed a guideline-based clinical decision support system, SIEGFRIED (System for Interactive Electronic Guidelines with Feedback and Resources for Instructional Educational Development), to promote guideline utilization. Our system: (1) delivers interactive, patient-specific guideline content to clinicians at the point of care; (2) supports multiple guidelines simultaneously through a centralized, Web-based server; (3) provides clinicians immediate access to educational/instructional resources on the Web; (4) expedites entry of guideline content into the server database using a guideline entry tool; (5) can integrate with computer-based health record systems; (6) is structured to import data from these systems; and (7) makes possible the development of centralized guideline resource centers for distributing multiple guidelines on a single system with a common interface for may user groups, permitting large scale implementation as well as efficient guideline modification and re-dissemination. SIEGFRIED has been populated with 25 guidelines. In field trials, clinicians have responded favorably to using SIEGFRIED.
