In October 1998, Michigan implemented a voluntary managed care program specifically designed for children with special health care needs (CSHCN) enrolled in Title V. This study investigates the impact of this program on health care utilization and costs and on family and provider satisfaction. For children who moved from the fee-for-service (FES) program into one of the two available managed care plans (Kids Care of Michigan), emergency department use decreases by over 20 percent, but the cost of health care services appears unaffected. Family and provider satisfaction is generally similar under both FFS and Kids Care. Families and care coordinators are satisfied with the care coordination elements available under managed care. In its first two years of operation, the lightly incentivized approach used in Michigan to serve CSHCN under managed care has a relatively small effect on the provisions of health care for this population.
