This Case Study describes an explosion in a cryogenic nitric oxide (NO) distillation process at the Isotec facility in Miami Township, Ohio. Remarkably, only one employee was injured. More than 2,000 local residents were evacuated for 24 hours. The key issues are: process hazards analysis, incident investigation, land use management, and emergency response.
