Care Management by Nurse Practitioner/Hospitalist Team Final rept. (July 1 2000-June 30 2004)

With the challenges of managed care capitation in academic medical centers the purpose of this study was to provide nurse/physician collaboration to enhance continuity of care, multidisciplinary team-based planning, expedited discharge, and assessment post discharge while reducing cost and not compromising quality. The aims of the Multidisciplinary, Doctor, Nurse Practitioner (MDNP) study were to describe the efficacy of a multidisciplinary team of hospitalist/attending physicians and advanced practice nurses on management of general medicine patients in a large academic medical center during hospitalization and 4 month post-hospital discharge on length of stay, costs and resource utilization, readmission, mortality, health-related quality of life, and patient satisfaction. The results from the intervention process variables will also be reported: communication/collaboration between physicians and nurses; survey of hospitalists activities by the attending physicians; telephone calls and home visits by the NPs; and attendance at the multidisciplinary team meetings.
