A fatal accident (the Accident) occurred on 12 August 2005 at approximately 0012 hours aboard the lift boat Myrtle, contracted to Forest Oil Corporation (Operator) while plug and abandon (P&A) operations were being conducted for the Operator on Lease OCS-G 5694, Main Pass Block 98, Well No. 3 (the Well), in the Gulf of Mexico, offshore the State of Louisiana. The fatally injured person was a crew member for a coil tubing unit (CTU) from Coil Tubing Services (CTS or the CT Contractor).
