This report covers the detailed review work done by the Editorial Unit of the Istituto Superiore di Sanita (ISS) on all the journals received through its exchange programme with the quarterly periodical Annali dellIstituto Superiore di Sanita. It gives a brief background of how the programme started and its evolution over the years, then covers the methods used to collect and analyse data followed by an evaluation of the results achieved. The first conclusion is that over the years the exchange programme has gone through a moderate but continuous decline from its heydays in the Seventies, when over 430 journals were exchanged, to the current 182. Of these, which represent 10% of the journals owned by the ISS, the majority come from domestic exchanges (43%), whilst 31% are from Europe and a further 26% from the rest of the world. The analysis of these data leads to the discussion of the usefulness of these exchanges and its possible role as a method of dissemination of scientific knowledge in an open access era, in particular for all those who have no access to the Web but deserve access to information. In this particular framework, a possible implementation would be auspicable.
