The purpose of this research was to better understand the magnitude, nature, and causation of adverse events (AEs) in ambulatory primary care and to design risk mitigation strategies that would yield demonstrable improvements in patient outcomes. The need to reduce risk for persons interacting with health care system is well recognized as a public health issue. AEs and risk factors for AEs have not been extensively investigated or characterized in the ambulatory care setting.
