Attempted Suicide During Treatment West Palm Beach VA Medical Center West Palm Beach, Florida. Healthcare Inspection

The VA Office of Inspector General Office of Healthcare Inspections conducted an inspection to review allegations that a high-risk patient was able to attempt suicide in the emergency department (ED) and again on the mental health (MH) unit at the West Palm Beach VA Medical Center (the facility), West Palm Beach, FL. The complainant also expressed concerns about staff training; poor communication with the family; and staff actions regarding an art therapy class, and the patients transfer to a non-VA treatment center. The purpose of the review was to determine whether the allegations had merit.
