Search of Fatality and Injury Records for Cases Related to Confined Spaces

Fatality and injury records pertaining to cases related to working in confined spaces (CS) were reviewed. A brief initial study was made to review data, find data sources, develop a profile of CS related characteristics, secure copies of CS related injury reports, and secure data from OSHA investigated instances of CS accidents. By far the most frequent accident type both in terms of events and in terms of the number of individuals hurt or killed was that which involved toxic atmospheric conditions in the CS. Far fewer were the injuries resulting from electrocution in CS, entrapment in unstable materials, or stresses from excessive exertion. The industries most frequently involved included the transportation equipment, construction, chemical, food, primary metallurgy, and paper industries. The data were reviewed with particular emphasis on whether or not certain guidelines were followed, specifically guidelines related to entry permits, atmospheric testing, use of personal protective equipment, lockout procedures, and correct rescue procedures. Recommendations were developed for additional guidelines which evolved from the study.
