The Clinical Applications of the Left Ventricular Assist Device.

The report covers the second full year of the Clinical Left Ventricular Assist Device program which was initiated on 11/3/75. Eight clinical LVAD implantations have been carried out to date,five during the current project year. Cases 4and 5expired after 30and 18hours of mechanical support due to continued postoperative hemorrhage and episodes of cardiac tamponade. LVAD function seemed satisfactory in both patients. Case 6,who underwent 120minutes of aortic cross-clamp time for an aortic valve replacement, mitral valve replacement and triple CABG,could not be resuscitated. The right ventricle failed to contract,so left-sided output was inadequate to fill the LVAD. Cases 7and 8,both with severe coronary artery disease,were satisfactorily resuscitated after 105and 100hours of circulatory support. Both patients are alive and in improved condition.
