The report presents an overview and documentation of the data file created from a survey of business establishments conducted as a part of the total study. The purpose of the study was to analyze the determinants of the decision by establishments to offer group health plans and describe the characteristics of establishments with and without plans and the employees who work in them. This report describes the scope of the survey, sampling procedures, method of collection, conduct of survey, response rate, number of completed interviews, sampling weights, content of the data set and layout of the data. It also includes the codebook, a questionnaire used in the survey and an SPD (summary plan description) abstraction form.
