The VSOP program developed a program of case management services to vulnerable older persons living in the community. This manual presents the systems that was developed--the approach and structure of case management, the organization of work and distribution of responsibility, and the procedures, methods and forms used in the work. The system is based on the work and experience of the case managers. The elements of the system are illustrated with anecdotes about clients and the work of case management. This manual was developed to help agencies adopt or adapt the case management system utilizing nonprofessionals (volunteers and paraprofessionals).
