Guidelines for Safe Maintenance Practices in Petroleum Refineries, Volume 2, Chapters 6-10

The recommended performance criteria for controlling maintenance hazards and suggested guidelines for safe work practices in petroleum refineries are reviewed. The criteria for authorizing maintenance on process equipment are discussed. A definition of the problem, including injury statistics and the causes of these injuries is given. The case histories of accident and near misses in industry are listed. The importance of hazard identification is discussed. The safety guidelines for authorization, preparation of work area, heat hazards, poor ventilation hazards, machinery hazards, cleaning, chemical handling, and equipment handling are reviewed. The authors recommend that an OSHA regulation which incorporates these performance criteria should not apply only to petroleum refineries, that additional investigation should be conducted to identify the affected industries, and that a program for safety during maintenance activities should include management commitment and a maintenance safety plan as outlined in this report.
