Addiction Severity Index and Instruction Manual (Fourth Edition)

The Addiction Severity Index is a five-page form for the recording of a brief structured interview designed to provide important information about aspects of a patient's life which may contribute to his or her substance abuse syndrome. It is the first step in the development of a patient profile for subsequent use by research and clinical personnel. A five-point rating scale is used by the patient to answer subjective questions in each of seven potential problem areas: Medical, Employment/Support, Alcohol, Drug, Legal, Family/Social, and Psychiatric. Severity ratings are also entered by the interviewer for each of the individual problem areas. Severity is defined as need for additional treatment, and is based on reports of amount, duration, and intensity of symptoms within a problem area. The 32-page instruction manual that accompanies the Index presents guidance in its use and item-by-item explanations of the individual segments of the Index.
