On April 11, 1985, a carpenter had attached a 4' x 8' metal form to a crane. The metal form was to be used in conjunction with a highway construction job. The victim was directing the form into place when the crane came in contact with an overhead power line. The victim was electrocuted and pronounced dead at a local hospital. A second carpenter located 75' from the victim was drilling holes in a plywood form with an electric drill. He was admitted to a hospital with severe burns. The Division of Safety Research (DSR) decided to include the case in the FACE program. A field evaluation is planned for April 16-18, 1985. The area director for Federal OSHA is totally supportive of the FACE evaluation and will share all pertinent information from their investigation.
