The PaCS instrument and procedures represent a synthesis of the best components of the modified survey approaches that HCFA has approved on an experimental basis in some states. PaCS codifies the preferred techniques used in conducting traditional surveys, techniques which have previously been employed only on an informal and inconsistent basis. The refinement of PaCS can produce a state of the art survey instrument which incorporates significant advances in terms of both sensitivity to, and sophistication in, measuring the outcomes of direct patient care. The PaCS Survey Report Form includes the following information collections: results of facility tour and general observations of environment and patient population, results of individual patient interviews and assessments, results of surveyor record reviews of patient assessment and care planning activities, and summary of above information which relates the PaCS findings to the appropriate conditions of participation.
