The six appendices in the volume are part of the larger report by the same title. Each appendix is referenced in appropriate places in the Volume 1 chapters. The materials in the appendices further document the research reported in Volume I to provide additional information that can aid others interested in learning more about the research. Appendix A provides short descriptions for each of the exercises developed under the contract. Appendix B is a compilation of the statistics obtained from the field tests of each exercise and exercise version; Appendix C consists of computer programs for four of the exercises that were prepared in a computer administered format, as well as in paper and pencil latent image formats; Appendix D is the new instructor's manual developed for the CSE self-contained self-rescuer (SCSR) under this contract. Appendix E is a copy of the script used in the videotape that accompanies each of the instructor's manuals for the new donning procedure for each of the four SCSR models; Appendix F is a copy of the Marvin R. Letcher first aid role play simulation exercise. These appendices are best used in conjunction with the research and evaluation results reported in Volume I to teach and assess problem-solving skills for coping with underground mine emergencies.
