Fault Tree Analysis of 615 Fatal Work Injury Events Associated with Electrocution, Confined Spaces, Explosions, Welding, and Power Tools

To assess the value of OSHA 36 information in the development of fault trees for industrial fatality or catastrophe producing events, 615 reports concerned with electrocutions, confined space fatalities, and fatal explosion, welding, and power tool incidents were analyzed. The results indicate that the reports contain valuable descriptive information that can be used to construct general fault trees for each of the five categories of incidents, but do not contain consistently enough information about each incident to allow construction of fault trees for individual incidents. The five categories of incidents investigated also were found not to be mutually exclusive; e.g., 76% of the welding incident reports indicated explosions. General fault trees for each of the five categories of incidents are included.
