ACMS Community Based Care Coordination Project for CCS Children and Their Families in Los Angeles County.

摘要

The goals of the project were to: (1) develop and implement an automated case management system in Los Angeles County to improve and enhance the existing California Children's Services (CCS) System; (2) establish a CCS Community-based care coordination program within Los Angeles County to coordinate and implement an individual service plan for children with special needs and their families; and (3) test the feasibility and desirability of these goals in Los Angeles County, California, and other states.
