National Ignition Facility Construction Safety Management Review

An accident occurred at the National Ignition Facility (NIF) construction site on January 13, 2000, in which a worker sustained a serious injury when a 42-inchdiameter duct fell during installation. Following the accident, NIF Project Management chartered two review teams: (1) an Incident Analysis Team to independently assess the direct and root causes of the accident, and (2) a Management Review Team to review the roles and responsibilities of the line, support, and construction management organizations involved. This report provides a discussion of the information gathered by the Management Review Team and provides a list of observations and recommendations based on an analysis of the information.
