Management Factors in Accident and Incident Prevention (Including Management Self-Evaluation Checksheets)

This document is designed to provide a means for accident/incident prevention by familiarizing managers with management-related accident/incident causal factors which have been revealed in Department of Energy (DOE) supported accident investigation research. It also helps to prepare managers for the DOE investigative process by making the manager aware of those factors which will be investigated in a formal DOE accident investigation. Checklists are provided to assist the manager in determining his and his organization's state of readiness relating to accident prevention. (ERA citation 09:001364)
