Nuclear Power Safety Reporting System, Implementation and Operational Specifications.

The report is the last in a series investigating the feasibility of adapting a voluntary, anonymous, non-punitive, third-party managed reporting concept in a U.S. commercial nuclear industry/regulatory environment. Such a system is intended for use in identifying and quantifying, in an uninhibited manner, the factors that contribute to the occurrence of significant safety incidents which elicit either positive or negative responses from humans in U.S. nuclear power plants. This report specifies the elements of a Nuclear Power Safety Reporting System (NPSRS), along with operating procedures and forms to be used for accepting, integrating and processing reports submitted to the system. Also included is a taxonomy for collating and storing reports received from a variety of sources addressing myriad safety-related topics.
