ROCKET, SMOKE. 3.5-INCH: WP, M30 (Lot RD-1-142) (MIF A-81-59)nThis malfunction investigation was begun as a result of a report by Camp McCoy, Wisconsin that the motor of a round of this ammunition had ruptured in its launcher, and split the rear portion thereof (Figure 3 and 4).n2. ROCKET, PRACTICE, 3.5-INCH: M29A2 (Lot COP-2-425) (MIF A-104-61)and (Lot COP-2-419) (MIF A-143-61)nFort Huachuca, Arizona (July 1961) and Fort Bragg, North Carolina (September 1961) experienced training accidents in which Rocket, Practice, 3.5-Inch: M29A2 exploded in the launchers.
