Diagnosis of Schistosoma Japonicum Infection. Areas of Uncertainty.

The most appropriate methods of making the diagnosis of Schistosoma japonicum infection are discussed. Examination of only one stool specimen is a particularly insensitive technique. Untreated patients with only serologic evidence of infection should receive praziquantel, even though serologic tests cannot currently distinguish between active and passive infection. This recommendation particularly applies to cerebral schistosomiasis. For research studies viable eggs must be found if active disease is being evaluated but serologic tests are more appropriate if a study is concerned with both active and older cases. (Author)
