Role of Monokines in Altering Receptor and Non-Receptor Mediated Vascular Contraction in Sepsis

Septic patients frequently exhibit hyperdynamic cardiovascular function that is characterized by elevated cardiac output and reduced systemic vascular resistance (SVR). The reduction in SVR occurs despite significantly elevated circulating levels of epinephrine and norepinephrine (NE) that occur in septic humans. The disorder in SVR is reflected by a vascular hyporesponsiveness to infusion of exogenous NE and angiotensin II in an in vivo animal model for sepsis; reduced responses to NE persist when isolated vascular tissue from septic rats is examined in vitro. In the present study, aspects of the relationship between immune system mediators and vascular function were examined. The data suggests that a persistently diminished vascular response to catecholamines, similar to responses observed in vascular tissue isolated from septic animals, can be specifically induced by the monokines interleukin 1 (IL 1) and tumor necrosis factor (TNF, cachectin).
