For years, the oil and gas industry has struggled to develop a cohesive tool for reporting, investigating and managing incidents. For some operators with legacy systems in place, the challenge of implementing a new system on a global scale has proved difficult, with the result that year in, year out, the same type of incidents would occur with the same frequency.
