A 5-year-old boy presented with a one day history of a generalized, pruritic, erythematous papular rash and facial swelling. The rash began as black macules with surrounding erythema on his right arm and spread to involve his face, trunk, and extremities. He had spent the previous day at a local beach and seaside park. He had a similar eruption following a prior visit to the same beach, which was treated symptomatically with topical corticosteroids with eventual resolution. He was otherwise healthy without food or drug allergies.
