Steroids and pediatric cardiac surgery: the right drug, at the right time, for the right patient.

With great interest, we read the recent article on adrenocortical response in infants undergoing cardiac surgery with cardiopulmonary bypass and circulatory arrest (1). We would like to comment on the conclusion by Gajarski et al that the subset of patients with elevated adreno-corticotrope hormone/cortisol ratios seems to have a clinical status consistent with adrenal insufficiency and might require moderate-to-high vasoactive sup-port and become a target group for early postoperative steroid therapy.
