We report a case of complete upward migration of a ventriculoperitoneal shunt into a subgaleal space in an infant and discuss the possible causes. This 7-month-old girl was born premature at 33 weeks gestation. She was noted to have a patent foramen ovale and transposition of the great arteries. Shortly after birth, she suffered a grade 2 intraventricular hemorrhage resulting in progressive ventriculomegaly and had a ventriculosubgaleal shunt inserted. Cardiac septostomy was done as her clinical condition would not permit major corrective cardiac surgery. She improved with these procedures and at 4 months of age underwent corrective cardiac surgery successfully.
