To the Editor: Nafamostat mesilate (NM) (6-amino-2-naphthyl p-guanidinobenzoate dime-thanesulfonate) is a synthetic protease inhibitor generated during the coagulation cascade as well as in the inflammatory process. This drug is mostly used for the treatment of acute pancreatitis and disseminated intravascular coagulation. Drug-induced liver injury (DILI; drug-induced hepato-toxicity), such as hepatocellular necrosis caused by DILI, caused by nafamostat mesilate is extremely rare. Pancreatico-biliary malfunction (PBM) is defined by the Japanese Study Group on PBM1 as a union of the pancreatic and biliary duct that is located outside the duodenal wall. We herein report a case of nafamostat mesilate-induced hepatocellular necrosis with pancreatitis associated with PBM involving a 3-year-old boy.
