Insulin autoimmune syndrome (Hirata disease) as differential diagnosis in patients with hyperinsulinemic hypoglycemia.

Insulin autoimmune syndrome (IAS), or Hirata disease, is a rare disorder characterized by hyperinsulinemia and fasting or late postprandial hypoglycemia associated with the presence of antibodies to human insulin in patients who have not been injected with insulin. We describe a 55-year-old male Brazilian white patient with a BMI of 33 kg/m2, complaining of repeated episodes of weakness, sweating, palpitations, hunger, and tremor in late postprandial period. He had no personal history of diabetes nor insulin usage.
