Although many operative methods have been proposed for the treatment of sacrococcygeal pilonidal sinus, no clear consensus as to optimal treatment has been reported so far.1 Recently, we designed a new, minimally invasive way to treat pilonidal sinus, video-assisted ablation of pilonidal sinus.2 We performed this procedure by using a 4-mm, continuous-flow operative hyster-oscope (Bettocchi Office Hysteroscope "size 4"; Karl Storz, Tuttlingen, Germany) with a 30° grade optic containing a 5-Fr working channel. Although Meinero et al3 designed a very similar minimally invasive technique using the Meinero fistuloscope with an 8° optic and a 7-Fr working channel (Karl Storz) ,4 we think that the Bettocchi Office Hysteroscope is the best for our technique. The 30° optic allows easy visualization of all of the cavity wall, permitting the identification of any lateral tracks to be destroyed and any hairs to be removed. Additionally, the very small diameter (2/4 mm) allows the easy entry of the endoscope through the external opening.
