We thank Professor Senn for his letter and agree that the terminology used in our article may have been confusing. We had hoped that our article was clear about the type of random effects being considered, having stated in the introduction that we 'do not consider the issue of treatment-by-centre interaction' [1], however, it would have been clearer to give an explicit model earlier in the article to avoid confusion.
