We read the article by Yoon and McCulley1 with keen interest. We wish to express the following comments to further refine the modification. The reconstruction of large upper eyelid defect is difficult and often challenging. Larger full-thickness defects frequently require the use of some form of flap and/or a free graft such as a Cutler-Beard flap that is a bridged full-thickness lower eyelid advancement flap, traditionally divided at around 6 to 8 weeks.2 We appreciate the authors for commendable modification to the Cutler-Beard procedure preventing eyelid retraction and entro-pion, the 2 common complications of classic Cutler-Beard procedure due to lack of posterior lamellar support.
