BP: Series of Errors Caused Macondo Blowout

BP said Wednesday it could find no single cause for the Apr. 20 blowout at its Macondo well that killed 11 workers and triggered a massive oil spill in the Gulf of Mexico. In- stead, “a sequence of failures involving a number of different parties led to the explo- sion and fire” the oil giant said in a report that contained the findings of its internal in- vestigation into the incident.
