Our specialty is in the midst of a monumental shift toward minimally invasive surgery. Despite keeping up with reading journals and practice bulletins, you obviously missed the memo that the vaginal approach was just too 20th century to have much of a role in the new millennium. We've moved on to complex straight-stick laparoscopy, robotics, and laparoendoscopic single-site surgery—hey, even natural-orifice operations are getting a look. A cursory glance at videos presented at national meetings is further proof that the minimally invasive train has left the station and is picking up speed. It stands to reason that, at this point in your career, you're just not going to catch it. Yet, what consistently gets short shrift in those videos is the importance of patient-doctor counseling about alternative treatments. What happened to the patients who weren't good "candidates" for the most innovative surgery and those unfortunate outcomes that no one wants to talk about?
