Persistent cough in a young smoker. Coccidioidomycosis infection, pulmonary nodular presentation.

A 34-year-old man, originating from the United Kingdom, presented with a persistent, mildly productive cough which he had for 1 month despite previous treatment with amoxicillin-clavulanic acid. His secretions were no longer colored and hemoptysis was never observed. He further complained of NYHA grade II dyspnea. Besides having smoked for 20 pack-years, his medical history was negative. Two years before, a routine chest radiograph was normal. His grandfather had died of lung cancer. The patient had lived in Belgium for the last 2 years, but he was working for an international express service company and needed to travel a lot through Europe and the USA.
