Training residents and fellows in the operating room challenges even the most experienced surgeons. When performing a surgery for the first time, the new surgeon often has difficulty focusing on more than one aspect of the surgery at any moment in time. When concentrating on the vitrector, the position of the light pipe is not foremost in the new surgeon's mind. Irreversible damage to the macula or optic nerve from an errant light pipe can occur before the precepting surgeon has a chance to intervene. To reduce the risk of iatrogenic light pipe damage during 25-gauge pars plana vitrectomy, we describe a simple method for fashioning a light pipe guard by modifying a 22-gauge Becton Dickinson Angiocath Autoguard intravenous (TV; Becton, Dickson, and Co., Franklin Lakes, NJ) catheter.
