Case 162: scapulothoracic dissociation.

A 50-year-old intoxicated man was brought to a level 1 trauma center after falling from his bicycle into oncoming traffic. At the scene, he was unresponsive, with hypotension and tachycardia. Intubation was subsequently performed. At initial evaluation in the trauma center, he continued to be hypotensive and tachycardic, with decreased breath sounds on the right side. A right-sided throacos-tomy tube was subsequently placed. He had swelling in his right upper chest and right shoulder and a pulseless right upper extremity. The following radiograph and trauma computed tomographic (CT) image were obtained.
