This book, written by a past BP safety engineer and a present one, is based on an internal BP report, prepared after the 2005 Texas City disaster as part of the programme to improve the company's safety record. It forms a good companion volume to the excellent BP Process Safety series of booklets on Hazards of Water, Hazards of Air, and similar subjects, written half a century ago by Amoco, recently updated and extended by BP and now published by IChemE.
