Varying accuracy of myocardial perfusion SPECT and perfusion cardiac MRI for the detection of critical LMS coronary disease

Single-photon emission computed tomography (SPECT) has been widely used for myocardial perfusion in clinical practice. It is a well-validated prognostic test for the assessment of patients with coronary artery disease (CAD). However, the diagnostic accuracy and predictive value of perfusion SPECT in left main stem (LMS) disease are less well documented. In one study, only 49 of patients with significant LMS disease had multi-vessel reversible perfusion defect on perfusion SPECT.
