A 13 year-old West Highland White Terrier bitch was presented for acute blindness and abnormal ocular appearance. Clinical examination revealed hypertensive uveitis associated with lactescence of the aqueous humour suggesting a hyperlipidaemia. This hypothesis was confirmed and a diagnosis of hypercorticism associated with secondary hypothyroidism and hyperlipidaemia made using ancillary examinations. There was rapid remission of symptoms and return of vision after symptomatic treatment of uveitis and ocular hypertension, in association with a low-fat diet. Specific treatment for the causal endocrine disorders maintains the animal clinically asymptomatic to this day. This report shows that initial ocular observations may represent the tip of the iceberg and the first signs of a global metabolic disorder.
