Hayat and his associates would make William Shakespeare [1] proud using equine technology, albeit from equine pericardium to treat a patient who presented with unstable angina and an expanding ruptured coronary aneurysm. As they emphasize in their case report, coronary artery aneurysms in the native arteries are relatively rare and present a clinical dilemma. The incidence is 1 - 1/2 [2] to 5% [3], and occurs more often in the right coronary artery [4]. Although rarer diseases can cause aneurysms, the majority of aneurysms are due to atherosclerosis [5]. They reference a series of nearly 1,000 patients [6] where there were no cases of rupture in this large clinical population.
