In this issue of CCI, Silva et al. provide an unusual and much needed insight into the outcomes of diabetic chronic kidney disease patients treated with renal artery stent placement . The landscape of this disease is vast. There are 8.6 million Americans over the age of 60 suffering from diabetes mellitus-more than 18% of this older population-and diabetes in 2001 was reported by the Centers for Disease Control as being responsible for 44% of all newly diagnosed cases of End Stage Renal Disease (ESRD) . Up to 10% of diabetics have RAS detected at autopsy , and diabetics with ESRD have the worse prognosis of any dialysis dependent patient group, with an annualized mortality of almost 30% . Even relatively modest 10 mm Hg reductions in systolic blood pressure in these individuals can reduce cardiovascular complications by 12% .
