A long-awaited report on the causes of last year's Gulf of Mexico oil spill disaster lays blame primarily at the feet of oil giant BP, but also faults contractor Halliburton and rig owner Transocean for contributing roles. The report also lists nearly 50 recommendations for enhancing the safety of offshore operations, including improvements to blowout preventers and gas-kick detection systems, as well as ways to mitigate the risk of ignition aboard a rig. Investigators presented their conclusions in a joint report released last week by the US Coast Guard and the Bureau of Ocean Energy Management, Regulation and Enforcement (BOEMRE).
