Encephalopathy as an initial symptom of rhabdomyolysis: a commentary.

Sir,I read the article by Zheng et al., with great interest. In this article the authors have presented a patient with encephalopathy and rhabdomyolysis and proposed that the encephalopathy is secondary to rhabdomyolysis. However, there are many factors which are not in favor of this diagnosis. A short history of five hours before admission and no preceding symptoms, maximum neurological deficit at the onset not coinciding with maximum creatine phosphokinase (CPK) levels and rapid improvement in neurological symptoms but persistent CPK rise contradicts the diagnosis. Further there is no definite sufficient preceding cause to explain the etiology for rhabdomyolysis. Possible etiology can be viral encephalitis with rhabdomyolysis.
