Prostate cancer in 2010: GSU: misclassification or biological progression?

Gleason sum upgrading (GSU) represents a common event in patients affected by organ-confined prostate cancer.1"3 Several studies from the past few years have examined the effects of GSU on the natural history of patients with prostate cancer, and predictors of GSU have been identified.12 However, several interpretations have been proposed with regard to the clinical relevance of GSU, possibly inflating its implications in daily clinical practice.
