Primary aldosteronism typically presents as resistant hypertension and hypokalemia and is the most common form of secondary hypertension, with a prevalence of 1-10% of hypertensive patients in referral centers.1 In the classic syndrome, these symptoms are attributable to an aldosterone-producing adenoma and resolve with surgical removal of the adenoma. However, other etiologies of primary aldosteronism have been described, including bilateral adrenal hyperplasia. Given the bilateral production of aldosterone in patients with this condition, unilateral adrenalectomy is unlikely to cure hyperaldosteronism.
