Is the central portal a safe approach for arthroscopy of the knee?

I read with interest the report by Bayar et al.. Recently, my colleagues and I submitted a similar study to Current Orthopedic Practice (in review), stimulated by the concerns regarding with the transtendineal arthroscopic portal. Several papers after the original publication by Gillquist and Hagberg have shown the central approach is a useful portal in knee arthroscopy. Eriksson, however, in a cadaveric study, found that this does not offer any major advantages compared with the standard lateral portal. In addition, this portal has been associated with increased postoperative pain, and decreased isokinetic quadriceps strength postoperatively. Complications, such as peritendinitis, tendinitis or tendon granuloma have been reported.
